Please search below for all procedures and claim forms in connection with both the General Business and Life Business Classes of Insurance. You can easily access and download all necessary forms for a speedier handling of your claims.

Before completing the necessary claim forms, please study the claims procedures in order that you are fully conversant with the forms you have to submit.

General Business Class

MOTOR

In terms of your policy, the notification of a claim is essential for informing the Company that a claim is raised by the policyholder or a third party whenever the insured motor vehicle is involved in an accident. The same procedure also applies in case of damage to the windscreen or the tyres.

In the event of an accident, please call 80004040
Inform the Road Assistance Officer of the exact location
Do not remove the vehicles involved unless it is necessary
Describe fully to the Road Assistance Officer how the accident occurred
Provide the Road Assistance Officer with your Driving License and Motor Vehicle Registration Certificate
Under no circumstances you should admit liability, make an offer, promise or payment without the Company’s written consent
In the event that the above procedure has not been followed, the policyholder should immediately submit in writing a claim form directly to the Company or via your insurance intermediary.

PROPERTY

LIABILITY

In the event of an incident that may give rise to a claim, please call 80004040

PERSONAL ACCIDENT

In the event of a personal accident, please submit immediately and in writing a claim form directly to the Company or via your insurance intermediary.
The Company’s Claims Department will communicate with you.

Life Business Class

The Policyholder:

should inform the Company in writing or notify the Health Line (Tel. 80005050) at least 36 hours before the admission of an insured person to a hospital
submit to the Company all necessary documents, medical certificates and information
inform the hospital and the attending physicians that the Company’s prior approval is required
agrees that that the notification for the payment of any expenses does not create any liability on the part of the Company
shall inform the Company within 24 hours from the admission of an insured person to a hospital for an emergency treatment
should submit to the Company within 20 days from the date the treatment commenced a completed Claim Form together with all original invoices and payment receipts shall pay all expenses in connection with a medical examination requested for by the Company related to the claim and conducted by medical practitioners appointed by the Company.
